You know more about the publishing world than I but I'm not sure I'd put blind faith in a 3rd-party to prepare a cover and print hundreds of copies without me even seeing it.
After enquiring into programme production at the club a few months ago, I do believe they put all their faith in a reputable firm who do the majority of club programmes in SL, so the blame should really be on them rather than the club.
As you say, not sure I'd want someone else putting out my publications without me seeing them first, if I were a designer I'd also be getting images from source - it'd be a lot cheaper as well as being incredibly more accurate.
Maybe things will change from next year...
