5th placed in the PL, plus the winners of the FA Cup and League Cup.
Birmingham have already taken the League Cup spot. If the winners of the FA Cup have already qualified through league position then their spot in the Europa League will go to the beaten finalists.
If Bolton beat Stoke in the semi-final, they will effectively take the last Europa League spot whether they win the FA Cup or not.
If Bolton do manage to win it and finish 5th in the PL, then the 6th place PL team will qualify as the third English representative.
